Where the Kidney Myth Came From
The myth likely began when early studies noticed that creatine supplementation increases creatinine levels in the blood.
But here’s the catch: creatinine isn’t the same as creatine, and higher creatinine doesn’t automatically mean kidney damage.
Creatinine is just a breakdown product of creatine metabolism — so if you take creatine, your body naturally produces more creatinine. It’s a normal physiological response, not a red flag.
A 2017 review by the Journal of the International Society of Sports Nutrition looked at years of data and found no evidence of kidney impairment in healthy individuals supplementing with creatine (Kreider et al., 2017).
What the Research Actually Says
Let’s look at what long-term studies show — because there’s a lot of them.
- In one of the most cited papers on the subject, researchers tracked healthy athletes using creatine for up to five years. Result? No adverse effects on kidney or liver function (Poortmans & Francaux, 1999).
- A randomized, double-blind trial on postmenopausal women found that creatine use not only improved muscle strength but also had no impact on kidney markers (Gualano et al., 2011).
- Even in clinical populations — such as people with type 2 diabetes — creatine showed no harmful changes in renal function when used responsibly (Gualano et al., 2010).
That’s decades of research, across men, women, and various health conditions — all pointing in the same direction: creatine is safe for your kidneys when used at recommended doses.
Who Should Be Cautious?
If you already have existing kidney disease, your doctor might advise against creatine — but that’s true for almost any supplement or medication that’s processed through the kidneys.
For everyone else? You’re in the clear. Just make sure you stay hydrated and stick to about 3–5g of creatine per day.
